| Specifications for
submitting artwork: Half-page is 7 x 5 portrait and quarter-page is 3 x 5
portrait. Artwork must
be received no later than August 15, 2004. |
Hotel Reservations: A block of rooms has been
reserved at the Columbus Airport Marriott Hotel at a special
group rate of $105.00 ($122.59, including tax). Reservations must be made on or
before September 30, 2004 to get the group rate; mention the Brain Injury Association of
Ohio when making your reservation. Check in time is 3:00 PM and Check out time is
Noon. Make reservations through the Marriott's toll free number (800-333-3333), or
through the hotel’s local reservation department (614-475-7551). Register early
(space is limited), especially to reserve a handicap accessible room. |

displays any time after 6:00 PM on Wednesday, October 13, 2004 Exhibitors assume
responsibility and hereby agree to protect, indemnify, and defend Columbus
Airport Marriott Hotel, its owners, operators, managers, or lessee together with its officers, directors,
shareholders, employees and agents: the Brain Injury Association of Ohio its trustees,
officers, employees, agents and members against all claims, losses or damages to property
of installation, removal, maintenance, occupancy or use of any part of the hotel’s
exhibition area unless any such liability was causes solely by the negligence or
recklessness of the hotel or its operators. |
