NeuroResource Facilitation

NeuroResource Facilitation is a free program offered by the Brain Injury Association of Ohio to help individuals who have sustained a brain injury, their families, caregivers, and the general community navigate resources and challenges after a brain injury.

We offer support and resource navigation to help an individual transition back to living, working, learning, and playing in the community.

Staff will work with individuals to:

  1. Assess current needs and goals - this person-centered process involves determining what is most important to the individual and what their individual needs are.

  2. Setting Goals and Creating a Plan - staff will work with the individual and caregivers to define goals and create a plan of how to achieve those goals.

  3. Resource Connection - staff will identify appropriate resources in the community to assist the individual in achieving their goals. Staff will help individuals and their caregivers access and navigate these resources.

  4. Follow Up - staff will continue to follow up and work with individuals and caregivers to ensure that they are able to work through their individualized plan to achieve their goals. This follow up includes providing ongoing education, support and assistance with accessing resources identified (filling out paperwork, making referrals and advocating within the system of care)


NeuroResource Facilitation can be accessed by:

  1. Participating in the BIAOH Transition Backpack program offered at some inpatient rehabilitation centers and hospitals.

  2. Referral from a participating brain injury clinic community partner.
